The State Board of Insurance adopts by reference Texas Casualty Dividend Disbursement Application, as applicable to motor vehicle insurance. This form may be obtained from the Deputy Insurance Commissioner, Casualty Group, State Board of Insurance, 1110 San Jacinto Street, Austin, Texas 78786.
